Custom Fields for Recipient Form & Trainer Form

Add extra information/ notes that might help Admins and Trainers understand a recipient/ training better

What are the Recipient and Trainer Forms?.

Recipient Forms are sent to the attendees of a certain training when they scan a QR code given to them during or after training. They need to fill it out to receive their certificate. Information like Name, Email Address, and Certificate/ badge name from the selected Collection shows up by default. Other information that is needed by the training organisation or trainer needs to be added by the Admin in the Account Settings under Trainer Module.

You can use Recipient Form Fields for collecting extra information from the recipients apart from what shows up as default like name and email. 

Trainer forms can be found when new training is created. This is added so that the Admin/ Trainer might want to take extra notes for later at the time of certain training.

Trainer Form Fields can be used for adding extra information in the certificate, almost like notes.

Note: Only Admins can add fields both on the Recipient & Trainer Forms.

Where to find the Recipient Form Fields? 

  1. Go to Account Settings and click on the Trainer Module.
  2.  You will find the Recipient Form Fields after Approval Types.
  3. Click on "Add new field."

4. As you can see below, you can add field names and descriptions. This helps the recipient understand what information is expected of them with a short explanation. Under Field Format, you can choose any one:

  • Input Field (type whatever is required)
  • Checkbox Field (click on multiple options)
  • Options Field (click any one option)

You can also choose if a certain field has to be filled.

Note: Don't add name and email address (recipients) fields as they show up by default on the form.
