Helps Admins collect necessary information from Recipients and Trainers during and after a training.
Recipient Forms
Purpose: Recipient Forms are designed to gather information from attendees of a training session. These forms are accessed by scanning a QR code provided during or after the training. Attendees must fill out the form to receive their certificate.
Default Information:
- Name
- Email Address
Additional Information: Any extra information needed by the training organization or trainer must be added by the Admin in the Account Settings under the Trainer Module.
Custom Fields: Recipient Form Fields can be used to collect extra information beyond the default fields.
How to Add Recipient Form Fields:
- Go to Account Settings.
- Click on the Trainer Module.
- Find the Recipient Form Fields section after Approval Types.
- Click on "Add new field."
- Add field names and descriptions to guide the recipient on what information is expected.
- Choose the Field Format:
- Input Field (free text entry)
- Checkbox Field (multiple options)
- Options Field (single option)
- Specify if a field is mandatory.
Note: Do not add name and email address fields as they are included by default.
Trainer Forms
Purpose: Trainer Forms are used by Admins/Trainers to take extra notes during a training session. These forms can be utilized to add additional information to the certificate, similar to notes.
How to Add Trainer Form Fields:
- The form fields for trainers can be found below the Recipients section in the Trainer Module (Admin view).
- Follow the same steps as for adding Recipient Form Fields:
- Go to Account Settings.
- Click on the Trainer Module.
- Add new fields as needed.
